Coverage Types: INDIVIDUAL - COUPLE - SINGE-PARENT - AND FAMILY.

OPTION ONE: BASIC HEALTH BENEFITS

100% reimbursement, no deductible, subject to $50,000 lifetime maximum

per insured for all services combined.

Alternative Practitioners, Psychologists, Physiotherapists, Speech and Hearing

Therapists, Accidental Dental Ambulance; Medical Equipment and Imaging,

Orthopedic Devices, Home Nursing Care, and Travel Coverage.

OPTION TWO: ENHANCED HEALTH BENEFITS

100% reimbursement, no deductible, subject to $50,000 lifetime maximum

per insured for all services combined.

Alternative Practitioners, Psychologists, Physiotherapists, Speech and Hearing

Therapists, Accidental Dental Ambulance; Medical Equipment and Imaging,

Orthopedic Devices, Home Nursing Care, and Travel Coverage. - PLUS Vision

Care.

OPTION THREE: with LIFE INSURANCE, AND ACCIDENTAL DEATH &

DISMEMBERMENT

OPTION FOUR: CRITICAL ILLNESS COVERAGE

Insured and Spouse have face amount of $10,000 for:

heart attack, stroke, cancer, multiple sclerosis, motor neuron diseases,

alzheimer's disease,  parkinson's disease, coronary artery bypass,

transplant of a vital organ, major organ failure, coma, paralysis, kidney

failure, blindness, deafness, dismemberment, loss of speech, major

burns, or a benign brain tumour

Supplemental Health Benefits

Prescription Drug Benefit

80% reimbursement for eligible prescribed medications

$5.00 per prescription co-payment

Annual maximum payable of $2,000

Hospital Coverage

100% up to $175 per day for a semi-private room in a public hospital

Dental Care Benefit

70% reimbursement with no deductible on diagnostic, preventive

services, basic restorations, periodental benefits, surgical services,

adjustment services, and denture services

Annual Maximum Dental Care Benefits

1st Year $500

2nd Year $750

Subsequent Years $1,000

Subject to a 3-month waiting period

Long Term Care Benefits

Long term care can provide a daily or monthly benefit to fund the cost of

nursing homes or home care.

DETAILS:

This is an affordable solution which protects savings, assets, and

financial security from the cost of long term care

Provides the flexibility to choose a suitable facility or receive

professional care in the home

Benefits are paid directly to the insured regardless of government aid

Benefits can be used to fund care or other out-of-pocket expenses not

covered by government, such as wheel chairs or physical therapy

Those already needing care can purchase an annuity or guaranteed income

plan to provide a specific monthly income.

BENEFITS:

$1000 to $4000 per month

Injury only, or injury and illness coverage

24-hour non-occupational coverage

Choice of elimination periods; zerio - 30 - or 120 days for occupational injury

and 30 or 120 days for illness

Loss of income benefit based on gross revenue or earned income

 

FEATURES:

Partial disability benefit - 50% for up to 180 days

Waiver of premiums after 30 days of total disability and when benefits are

available

Return to work assistance benefit, rehabilitation and financial assistance in

returning to work

$10,000 accident medical benefit, providing payment for medical expenses not

covered by provincial health plans, including physician, surgeon, and

registered nurse fees
